Established in 1991, Danila Dilba Health Service (DDHS) is an Aboriginal community-controlled organisation committed to providing culturally appropriate, comprehensive primary health care services and community development within the Biluru (Aboriginal) community of the Yilli Rreung (Greater Darwin) Region in the Northern Territory.
About the Role
The Support Worker Aged Care delivers high-quality, culturally secure care to Elders across centre-based, community, and in-home settings, providing practical care supports to enable Elders to remain at home living an active, self-determined and meaningful life.
Services are delivered in accordance with program guidelines ensuring adherence to the Aged Care Code of Conduct and Strengthened Aged Care Standards.
Responsibilities
Provide high-quality, person-centred care to elders in their homes, communities and in group settings.
Support elders with daily living activities in accordance with their individual care plan.
Promote independence, dignity, and cultural safety in all interactions, ensuring trauma-aware and healing-informed practices that meet the needs of Aboriginal and Torres Strait Islander elders.
Provide companionship, encourage social engagement, and support clients in maintaining community connections and emotional wellbeing.
Ensure all interactions with clients and staff show appropriate responses to their needs and demonstrate the application of DDHS code of conduct.
Qualifications
- Certificate III in Aged Care or Disability or Individual Support, or willingness to obtain.
- Demonstrated ability to communicate effectively with Aboriginal and Torres Strait Islander persons, including respect and understanding of client issues, cultural appropriateness and anti-discrimination.
- Knowledge of the social determinants of health (environment, economic and social inequalities) that impact the health and well-being of Aboriginal and Torres Strait Islander people.
- Proven teamwork, communication, and interpersonal skills with the ability to constructively negotiate outcomes with a diverse range of people and service providers.
- Commitment to continuous improvement and the delivery of safe and high-quality care and support.
- Genuine empathy and passion for helping others.
- Experience working in community aged care.
- Knowledge of WH&S; regulations relating to cleaning and chemicals.
- Demonstrated ability to work to a plan unsupervised.
- Willingness to use own vehicle in accordance with DDHS Grey Fleet Policy.
Employment Is Subject To
- Willing to undergo a Police Check.
- Ability and willingness to obtain a Working with Children Clearance (Ochre Card).
- Current driver's licence.
- Current CPR certificate or preparedness to gain one within 6 months of commencement.
- Fully vaccinated against COVID-19 or an exemption as listed on the Australian Immunisation Register (AIR).
- Full rights to work in Australia.
