Remuneration: Dependent on Qualification

* Clinical Psychologist: $57.30 - $69.84 per hour
* Occupational Therapist Level 4: $63.81- $65.35 per hour
* Social Worker Level 4: $63.81- $65.35 per hour

In addition to base salary, all roles attract 12% Superannuation + Salary Packaging

Location: SWSLHD District Wide Service - Based at Liverpool Hospital Mental Health Service

Join the Pathways to Community Living Initiative (PCLI) at South Western Sydney Local Health District and help support people with severe and persistent mental illness (SPMI) to transition safely from long‐stay inpatient care back into the community. In this role, you'll work with inpatient and community teams to coordinate assessments, plan safe discharge pathways, and ensure consumers receive recovery‐focused, strengths‐based care. You'll provide both leadership and direct clinical support while contributing to service improvement across the district. You'll also be part of a supportive statewide network driving better outcomes for people with complex mental health needs. Full‐time is preferred, but part‐time or job‐share arrangements can be negotiated.

Selection Criteria
To help us assess your suitability for the role, please tell us as much as you can about yourself in response to the questions below.

* Relevant qualifications as an Occupational Therapist or Clinical Psychologist currently registered with the Australian Health Practitioner Regulation Agency (AHPRA) or Completion of an Australian Psychology Accreditation Council accredited postgraduate Clinical Psychology qualification of no less than two years full time duration or recognised equivalent, or qualified Social Worker eligible for registration with the Australian Association of Social Workers (AASW).
* Demonstrated extensive experience in mental health rehabilitation providing advanced evidence based, strengths focused, recovery orientatedassessment and treatment for consumers with severe and persistent mental illness (SPMI) and complex needs.
* Demonstrated advanced interpersonal skills (including excellent negotiation and advocacy), problem solving skills and personal resilience and highlight how these skills have been vital and used when working with numerous internal and external stakeholders.
* Demonstrated experience in leading the planning, implementation and evaluation of local service delivery through quality improvement projects or research. Facilitation of projects that has demonstrated improvements in MH outcomes and showed sustained and improved change of practice and service delivery.
* Ability to work independently and in consultation with the multidisciplinary team and external stakeholders.
* Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
