At the Healthscope Independence Services (HIS), we are seeking a passionate and experienced Attendant Care Coordinator to lead our Specialist Disability Accommodation (SDA) apartment services operating within the NDIS.

* Permanent, Full Time
* Remuneration: In line with SCHADS Level 5 (pay point will be determined based on the experience)
* Primary office location Victorian Rehabilitation Centre, Glen Waverley

About The Role

This key leadership role will report to the Area Manager. The Access Coordinator is responsible for leading a team of Disability Support Workers to deliver high‐quality, person‐centred services for clients across both private and HIS residential houses. This role plays a key part in ensuring that all supports are delivered safely, effectively, and in alignment with individual client needs and organisational standards.

A Typical Day May Include

* Reviewing staffing levels and adjusting rosters to meet client needs and budget requirements.
* Responding to client or family enquiries and resolving issues promptly.
* Supporting residential supervisors with operational or staffing challenges.
* Monitoring compliance and quality, such as staff training, documentation, audits, and continuous improvement activities across the service.
* Conducting check‐ins, coaching sessions, or performance discussions with Disability Support Workers.
* Completing incident reviews, risk assessments, or quality improvement tasks.
* Coordinating client admissions, transfers, or service changes.
* Participating in leadership meetings, reporting KPIs, and planning service improvements.

To Be Considered For This Role You Will Have

* Experience in disability, attendant care, or community care coordination
* Strong understanding of NDIS, TAC or iCare programs
* Experience coordinating teams across multiple sites, including rostering, workforce planning, and service delivery oversight.
* Excellent communication, written, interpersonal, and organisational skills.
* Excellent organisational and rostering skills
* Confident communicator with a proactive, solutions‐focused approach
* Ability to manage multiple clients and priorities
* Experience supporting complex or high‐needs clients is highly regarded
