**Key Responsibilities:**
1. Conduct comprehensive assessments to identify the needs of older adults, ensuring all assessments are conducted in line with Department of Health contractual requirements.
2. Facilitate referrals to essential services that support independent living, working closely with families and carers.
3. Promote the retention and maximization of function and capability in daily living, actively supporting older adults to live independently with dignity.
4. Engage with stakeholders, recognizing them as key partners in delivering high-quality care and services.
**Skills and Qualifications:**
* Tertiary education in a health-related discipline (e.g., nursing, occupational therapy, physiotherapy, social work).
* At least 1 year of experience in aged care, including client assessments and/or service delivery.
* Current unrestricted General Registration with AHPRA or relevant professional association.
* Proficiency in Word, Excel, Outlook, and client management systems.
