Yellow Bridge Canberra ACT | Hybrid & Community Part-Time – 3 Days per Week (with pathway to full-time)
Salary: $110,000 – $120,000 + Super (pro rata, depending on experience)
Yellow Bridge is a values-driven NDIS provider supporting 500+ participants across ACT, NSW, Tasmania and Victoria. We specialise in psychosocial disability, complex support needs, housing instability, forensic presentations and system navigation.
Our model is integrated — bringing together Support Coordination, Specialist Support Coordination, Recovery Coaching and Social Work under one umbrella. We are expanding our Social Work capacity and are seeking a confident, systems-aware clinician who thrives in complex environments. About the Role
This is not a generic counselling role.
You will work with participants experiencing:
• Severe and persistent mental health conditions
• Housing instability and SIL transitions
• Justice system involvement
• Trauma histories and systemic disadvantage
• Multi-agency complexity
Your role will include:
• Delivering one-on-one therapy using evidence-informed, recovery-oriented approaches
• Providing case management and system navigation support across housing, health, justice and community services
• Delivering Capacity Building supports under NDIS (Improved Daily Living / CB Social & Community Participation)
• Conducting psychosocial and functional assessments
• Completing high-quality NDIS progress & plan review reports
• Developing risk management and support plans
• Advocating across systems to reduce barriers and improve outcomes
• Working collaboratively with OTs, SCs and Recovery Coaches in complex cases
This role begins at 3 days per week, with a strong opportunity to transition to full-time as demand continues to grow.

What We’re Looking For
• Degree-qualified Social Worker
• Eligibility for AASW membership (or working towards)– strong preference to AASW Accredited Social Worker
• Strong report writing skills (essential)
• Solid understanding of NDIS legislation and reasonable & necessary criteria
• Experience in psychosocial disability, therapy and complex case management
• Confidence navigating systems and advocating for participants
• NDIS Worker Screening & relevant checks
