Home Care Liaison
We are seeking a motivated and passionate Home Care Liaison to join our team.
As a key member of our care team, you will be responsible for delivering quality in-home care and assistance to our growing client base. This includes working with clients, their families, staff, and stakeholders to ensure services are provided in accordance with client needs, goals, and preferences.
Your key responsibilities will include:
* Building strong connections with clients and their families
* Assigning staff to clients based on qualifications, availability, and client preferences
* Organising support for clients with disabilities and monitoring progress
* Referring clients to agencies providing additional help
* Assessing client needs and resources for health, welfare, and other facilities and services
* Responding promptly to emails, enquiries, and requests from clients, staff, and management
* Maintaining accurate records of incidents and feedback
* Re-organising work priorities to meet the needs of a fast-paced environment
* Supporting clients to meet their goals
* Conducting Client Care Assessments and Care Planning
* Supervising and managing Care Staff
You will also discuss and promote our services with medical professionals and referral sources, and report to head office.
To be successful in this role, you will have:
* Experience in assessing and planning care needs
* A sound understanding of the Aged Care and Disability systems
* A strong client focus based on their goals, preferences, and needs
* Demonstrated experience managing staff
* Current National Police Check and Working with Children Check
* A Full Driver's Licence
* The right to work in Australia
* Excellent organisational, administrative, and time management skills
