Join to apply for the Physiotherapist - Raceview role at Southern Cross Care Queensland (SCCQ). This full-time Physiotherapist position is based at St Mary's Residential Aged Care Home in Raceview and focuses on delivering high-quality, person-centred physiotherapy services for residents and Home Care clients in the Ipswich area.
The role aims to promote mobility, functional independence, pain management, fall prevention and an overall improved quality of life for residents and clients.
Responsibilities
 * Provide skilled, holistic mobility, strengthening and falls prevention assessments, consultation, history taking, diagnosis and treatment services.
 * Support face-to-face consultations with remote telehealth reviews as required for remote referrals.
 * Prescribe and implement physical assessments to enhance resident and client mobility needs and follow up on appropriate actions.
 * Monitor progress and review care as needs change; follow up with residents to ensure positive health outcomes.
 * Refer residents to other providers when needs exceed the PT scope of practice.
 * Coordinate care with General Practitioners and other Allied Health professionals to deliver optimal care and build collaborative relationships based on trust and professional respect.
 * Document all care provided and education given within the health records as per professional and company standards.
 * Support residents and clients in fulfilling their choices and preferences.
 * Participate in SCCQ Clinical Governance and Continuous Quality Improvement objectives to maintain good clinical practice.
What you will bring to the role
 * Bachelor degree and AHPRA registration as Physiotherapist
 * Strong clinical reasoning, assessment and decision-making skills
 * Broad scope of practice including diagnosis of acute mobility, dexterity, pain and rehabilitation conditions in older people (desirable)
 * Experience in aged care, rehabilitation or community health (desirable)
 * Commitment to evidence-based practice and continuous quality improvement
 * Excellent communication and interpersonal skills with empathy and respect for older people
 * Ability to work collaboratively in a multidisciplinary team
 * Professional Liability Insurance as required for practicing physiotherapists
