The Hobart Continuing Care Team is responsible for delivering community mental health care to adult consumers in the Hobart metro and southern surrounding areas. The team works closely with all associated mental health teams, the Royal Hobart Hospital, families, Primary Health, Community Sector Organisations, and the National Disability Insurance Scheme.

The Role:

You will act as a clinical expert in the area of practice, providing clinical resources for nursing, medical and allied health staff in managing consumers with complex mental health needs. You will work collaboratively with other members of the multidisciplinary team to deliver comprehensive care within the designated area of practice.

* Develop and implement evidence-based practices to improve outcomes for consumers

* Provide clinical leadership and support to team members

* Collaborate with stakeholders to develop and maintain effective relationships

* Maintain accurate records and reports

Required Skills and Qualifications:

To be successful in this role, you will need:
">
">
* A Registered Nurse qualification with specialist tertiary graduate or postgraduate mental health/psychiatric nursing qualification
">
* Comprehensive knowledge and understanding of relevant legislation and awards
">
* Highly developed interpersonal skills, including written and oral communication, negotiation and conflict resolution skills
">
* Ability to work constructively as part of a multidisciplinary team at both service and community levels
">
* Current Driver's Licence
