Support Worker Co. is recruiting Intensive Family Support Workers for an innovativepilotproject based in Bundaberg.
This role involves working directly with vulnerable families, addressing complex challenges, and helping them achieve safety, stability, and resilience.
Key Responsibilities
* Implement individualised family support plans under the direction of a Service Coordinator, addressing a range of behavioral, psychosocial, and safety needs.
* Provide hands-on support for each family, including 2-3 weekly face-to-face sessions focused on behavior management, trauma recovery, and parenting skill development.
* Foster family connections to specialised services and essential resources such as healthcare, housing, and education.
* Monitor family progress and maintain detailed case documentation, ensuring family goals are consistently met.
* Adapt to flexible scheduling based on each family's needs to maximise engagement and support outcomes.
What We're Looking For
* Intensive family support experience, particularly in areas such as crisis intervention, mental health, and family crisis situations.
* Strong documentation and case management skills to capture the family's journey and support plan outcomes.
* Flexibility and crisis intervention skills to respond effectively to each family's unique challenges.
* Collaborative communication skills to liaise with Service Coordinators, stakeholders, and team members.
* A goal-oriented approach focused on empowering families for sustainable success.
* Work independently with each family to create a support schedule that suits everyone.
* You have open availability that will allow you to work up to 38 hours a week with 3 to 4 families.
What you need:
* Certificate IV in Community Services or relevant
* Previous experience supporting children and families with complex needs.
* Australian driver's license
* First Aid Certificate with current CPR
* Working with Children Check (paid employee status)
* Have a reliable car with registration.
* Reliable smart phone with data
