Social & Community Access Disability Support Worker

Role:

A Social and Community Access Disability Support Worker provides person-centred support to people with disabilities, helping them access the community, build independence, and achieve their NDIS goals. This role focuses on supporting participants to engage in social, recreational, educational, and daily life activities outside of the home while promoting choice, inclusion, and confidence.

Key ResponsibilitiesCommunity Participation

Support participants to attend social outings, appointments, shopping trips, recreational activities, education, and community events to encourage independence and reduce social isolation.

Transport Assistance

Provide safe and reliable transport to appointments, programs, community activities, and other scheduled supports while ensuring participant comfort and safety. Company vehicles will be provided for this.

Skill Development

Assist participants in developing life skills such as budgeting, shopping, using public transport, communication, time management, and confidence in social settings.

Emotional and Social Support

Build positive and trusting relationships, provide encouragement, companionship, and emotional support to improve participant wellbeing and confidence.

Goal-Focused Support

Work alongside participants to support their individual NDIS goals, encouraging personal growth, independence, and active community involvement.

Behaviour Support

Implement behaviour support plans where required and provide calm, respectful, and consistent support for participants with complex or challenging behaviours.

Safety and Wellbeing

Monitor participant health, safety, and wellbeing during shifts, respond appropriately to incidents or emergencies, and report concerns promptly.

Documentation

Accurately complete shift notes, incident reports, progress updates, and communicate effectively with team members, families, and management.

Other Duties

* Attend staff meetings and ongoing training as required
* Work collaboratively with families, allied health professionals, coordinators, and support teams
* Maintain compliance with NDIS Practice Standards and organisational policies
* Promote participant dignity, choice, privacy, and independence at all times
* Clean vehicle after you use it
* Assist participants with cleaning the area they were in

Key Qualities and Requirements

MUST live within 50km of Traralgon, VIC 3844

* Compassionate, patient, and reliable
* Strong communication and interpersonal skills
* Ability to work independently and as part of a team
* Flexible availability, including weekdays, evenings, weekends, and public holidays if required
* Strong understanding of person-centred care
* Commitment to empowering people with disability
* Ability to speak and comprehend English well
* Professional presentation and strong work ethic

Qualifications and Compliance

* Current Driver’s Licence and reliable insured vehicle (essential)
* NDIS Worker Screening Check
* Working With Children Check
* First Aid and CPR Certification
* National Police Check (preferred)
* Certificate III in Individual Support (Disability) or actively working towards this qualification. We are NOT looking for people with a Certificate IV.
* Experience in disability support, community access, mental health, or similar roles preferred but not essential.

Pay: $31.76 – $43.08 per hour
