Remuneration: Dependent on Qualification

* Clinical Nurse Consultant Grade 1: $2,501.49 - $2,552.60 per week
* Occupational Therapist Level 3: $2,240.23 - $2,312.82 per week
* Physiotherapist Level 3: $2,240.23 - $2,312.82 per week
* Psychologist: $1,541.85 - $2,256.76 per week
* Social worker Level 3: $2,240.23 - $2,312.82 per week
* Speech Pathologist Level 3: $2,240.23 - $2,312.82 per week

In addition to base salary, all roles attract 12% Superannuation + Salary Packaging

The Disability Response Team (DRT) plays a crucial role in improving longstanding disparities in hospital experiences for people with disability, particularly as South Western Sydney has one of Australia's fastest growing populations of NDIS participants and increasing complexity in disability related care. In this role, the Disability Resource Coach works collaboratively with multidisciplinary teams and external stakeholders, including the NDIA discharge pathway to identify and address disability related needs, establish reasonable adjustments and support effective discharge planning. The position strengthens coordination and continuity of care between hospital and community settings, reduces avoidable hospital presentations and extended stays and implements proactive care navigation strategies for individuals with unmet or unstable supports. By providing guidance, leadership and capability building across SWSLHD services while fostering strong internal and external partnerships, this role directly advances equitable person-centred care and delivers meaningful outcomes for people with disability across the district.

Are You the Right Fit?
The ideal candidate is a collaborative and compassionate professional with experience supporting people with disability in complex health and community settings. They work confidently within multidisciplinary teams and external services to improve care experiences, ensure safe coordination between hospital and community care and support effective discharge planning. They understand disability related needs, can navigate NDIS processes and can guide staff in establishing reasonable adjustments and appropriate care pathways. The right person demonstrates leadership, contributes to quality improvement and education and uses evidence informed practice to support clinical decision making. They build strong relationships across SWSLHD and partner organisations, manage complex issues through effective engagement and escalation processes and provide short term care navigation for people with unmet or unstable supports. Adaptable and committed to equitable care, they uphold CORE values and ensure people with disability receive safe, coordinated and person-centred support across the district.

Selection Criteria
To help us assess your suitability for the role, please tell us as much as you can about yourself in response to the questions below.

* Relevant qualification/ registration in one of the disciplines, as noted in the essential criteria (please refer to the position description)
* Demonstrate your in-depth understanding of the health and disability sectors as a Health Clinician supporting clients within the National Disability Insurance Scheme (NDIS). Please provide examples of how you have coordinated care and navigated systems for individuals with complex needs—particularly those exhibiting behaviours of concern—across multiple services and agencies
* Describe your understanding of the principles of access and reasonable adjustment for people with physical, cognitive, communication, psychosocial, and sensory disabilities within healthcare settings. Please provide specific examples of how you have identified barriers and implemented practical adjustments to ensure equitable participation and inclusion of patients or consumers throughout their healthcare journey.
* Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
