About the role
The Community Support Worker provides essential in-home and community-based support services to older people, enabling them to maintain their independence, enhance their well-being, and remain connected to their community. This role focuses on delivering person-centred care and support in line with individual care plans and aged care standards.
KEY RESPONSIBILITIES
Direct Client Support
Assist with domestic tasks, including housekeeping, laundry, and meal preparation.
Support clients with mobility, transfers, and transport to medical appointments, shopping, or social outings.
Offer companionship and encourage social engagement to reduce isolation and improve quality of life.
Provide personal care (e.g., showering, dressing, grooming, medication assistance) in line with care plans and client preferences.
Monitor and report changes in a client's condition or needs to the Care Coordinators.
Encourage, motivate, and assist clients to maintain their personal independence.
Respond to incidents and administer first aid if required.
Compliance & Documentation
Maintain accurate and timely records of services provided using relevant digital systems or documentation tools.
Follow workplace health and safety procedures, infection control measures, and manual handling guidelines.
Teamwork & Communication
Collaborate with team members, health professionals, and families to provide holistic support.
Maintain professional and respectful communication with clients, families, and stakeholders.
Participate in team meetings, training, and continuous professional development.
General Responsibilities
Uphold Awabakal's Code of Conduct, Core Values, policies, and procedures.
Ensure compliance with the Aged Care Code of Conduct and Aged Care Quality Standards.
Demonstrate and uphold exceptional safety standards, ensuring personal and client safety in accordance with workplace health and safety requirements.
Display a positive attitude and foster effective working relationships with internal and external stakeholders.
About you
Holds a Certificate III in Individual Support / Assistant in Nursing / Home and Community Care / Aged Care (desirable)
Previous experience working in the health/aged care/disability sector
Genuine desire to provide the best quality support for people
Has a strong understanding of privacy and confidentiality issues
Demonstrates respect and understanding of the health and social issues impacting upon Aboriginal and Torres Strait Islander people in the area
