Join the collaborative and forward-thinking Hospital in the Home, Chief Operating Officer unit at CHHHS and play a key role in improving health outcomes across regional, rural, and remote communities. As the Social Worker Senior you will provide care in a patient's permanent or temporary residence for conditions that require ongoing clinical governance, monitoring and/or treatment, as a substitution for in-hospital admitted care.

About your opportunity
* Deliver high level social work clinical services in accordance with the Australian Association of Social Workers (AASW) Code of Ethics and AASW professional practice standards using highly developed assessment, intervention and problem solving skills which reflect evidenced based practice, initiative and innovation and are undertaken with a high degree of independence.
* Exercise independent clinical judgement and more complex decision making in areas such as psychosocial assessments; short and long term case management; counselling; crisis intervention for a broad range of clinical.
* Provide clinical support and education to less experienced staff as well as students and multidisciplinary staff whilst contributing to education and professional development programs. presentations.
About your role
* Lead and manage staff by fostering and committing to patient safety and quality in the delivery of health care by maintaining and evaluating safety and quality practices and initiatives.
* Provide clinical leadership and demonstrate high level management skills in monitoring and prioritising workload demands.
* Act as reference point and contribute to staff development through clinical practice supervision of staff and students, performance appraisal, professional development, recruitment and selection and mandatory training.
* Provide leadership in workforce, operational and quality planning.
