OverviewThe role is a new approach to support transitional care and is based in our community service department with close ties to acute inpatient services.
It will involve both inpatient and community-based care, including remote care.Elderly patients frequently experience longer lengths of stay and more frequent adverse events when admitted to hospital.
Geriatrician review can help to ensure that elderly patients are cared for in the most appropriate setting, whether this be an acute inpatient admission; direct subacute admission; ambulatory admission; or at home with community supports and rapid access to follow up.Located in Victoria a 1 hour flight from Melbourne.
