**Our Journey**
The Intensive Community Treatment (ICT) team serves as a vital alternative to hospitalisation for older individuals experiencing acute mental health challenges within the Older Adult Mental Health Service. The program operates with a threefold objective:
- Providing older individuals with an alternative treatment setting to hospitalisation during acute phases of mental illness, aiming to prevent hospitalisation whenever possible.
- Offering intensive treatment within the older person's home, aligning with the expressed wishes of both the individual and their family or caregiver during acute phases of mental illness.
- Minimising the duration of inpatient unit stays by delivering Assertive Community Treatment during acute episodes of illness.
Recently, the ICT program has expanded its hours of operation to provide an extended service from 8:30am until 8 pm, enhancing accessibility for individuals and families. Located in Endeavour Hills, ICT covers the entire Monash Health Mental Health Catchment Area. An exciting position is now available for a Grade 3 Social Worker to join the intensive Community Team, based in Endeavour Hills on a full time permanent ongoing role.
**A Day in the Life**
As the Senior Social Worker, you would hold a pivotal role within the Older Adult Intensive Community Treatment Team, leveraging expert knowledge to establish clinical judgements that are recognised as authoritative. As a clinical leadership role, you will harnesses their expert and extensive knowledge in mental health legislation to achieve and surpass service expectations and performance indicators.
As a specialist in Social Work, you will provide expert psychosocial assessments and play a crucial role in offering discipline-specific support and advice to the team, serving as an important resource to the team in matters relating to Social Work. You will also encompass and promote social work values such as social justice, respect for persons and professional integrity. The Senior Social Worker will also be expected to act into the Team Manager role when required.
The Grade 3 Social Worker will use highly specialised knowledge of theories and evidence to assist patients/consumers, families and/or carers/significant others with psychosocial support and advocacy through psychosocial assessment, intervention, counselling, discharge planning, information provision and facilitates access to community services
**What you need**:
- Bachelor of Social Work or Masters Social Work Qualifying OR Masters of Social Work, OR equivalent as assessed and approved by the Australian Association of Social Workers (AASW)
- Written evidence of eligibility for AASW membership for overseas qualifications is required
- 7 years of post-qualifying experience
- Accredited Social Worker and/or Accredited Mental Health Social Worker: As approved by the AASW (Desirable)
- Completion of a post graduate qualification in relevant field or working toward Desirable)
