Job Summary:
We are seeking a skilled Physiotherapist to join our Transitional Aged Care Program team. The successful candidate will work closely with a multidisciplinary team to deliver evidence-based physiotherapy care and support older adults transitioning from hospital to home.
About the Role:
* Deliver high-quality physiotherapy services to patients in a community setting.
* Collaborate with other healthcare professionals to achieve client-centered rehabilitation goals.
* Develop and implement comprehensive care plans tailored to individual patient needs.
* Maintain accurate records of patient progress and outcomes.
* Stay up-to-date with industry developments and best practices through ongoing professional development.
Required Skills and Qualifications:
To be successful in this role, you will need:
* A degree in Physiotherapy or equivalent qualification.
* Current registration with the Australian Health Practitioner Registration Agency (AHPRA).
* Demonstrated experience working in aged care and/or rehabilitation settings.
* Strong communication and interpersonal skills.
* Ability to work independently and as part of a team.
