We are seeking an experienced Behaviour Support Practitioner at the Proficient or Advanced level, committed to delivering high-quality, evidence-based, person-centred, and outcome-focused positive behaviour support to individuals.
 * Key Responsibilities:
 1. Assist individuals, along with their families and carers, to identify needs through assessment and planning.
 2. Develop, evaluate, and deliver evidence-based support plans within complex environments.
 3. Create meaningful goals and practical strategies using a positive behaviour support framework, aiming to eliminate restrictive practices in home and community settings.
 4. Evaluate plans regularly to ensure they reflect participants' goals, passions, and strengths.
 5. Build capacity and strengths of those supporting individuals, including family, carers, and friends.
 6. Collaborate within a multidisciplinary team, including mentoring less experienced practitioners.
