 
        
        An exciting opportunity has become available within BRHS Planned Activity Group (PAG) team for a motivated and experienced skilled to provide leadership in the delivery of our valuable Planned Activity Group Program. 
This position offers the perfect balance of professional fulfilment and personal wellbeing, with a Monday to Thursday schedule.

The BRHS Planned Activity Group Program provides meaningful activities to clients promoting socialisation, community inclusion and independence. 
Activities are focussed on meeting individual needs using person centred care planning and identified goals. 
The position: Ongoing, Part-Time, 64.0 hours per fortnight 
As the PAG Coordinator, you will support the delivery of quality, person centred programs for participants with a wellness and reablement approach. 
You will be responsible for the day-to-day planning of activities and programs, care planning and onboarding of new participants. 
You will support Grade 1 and 2 lifestyle assistants to deliver activities in line with program planning. 
This role will focus on quality of the program and ensure high levels of consumer experience and engagement.

Key Criteria Required: 
* Minimum Certificate IV in Leisure & Lifestyle/ Individual Care/ Disability. 
Diploma preferred 
* Two years team leader (or equivalent) experience in Aged Care or Disability services 
* Demonstrated knowledge and understanding of the application of Aged Care Standards, HACC & NDIS in developing Planned Activity Group Programs for eligible participants. 
* Highly developed interpersonal skills including exceptional communication (verbal and written), with a wide range of people, including co-workers, participants/carers, other health professionals and service providers
