About the role

This role is responsible for case management within a therapeutic and trauma informed model of care. It is responsible for carer and development of caregivers, caregiver assessment, contribute to overall service development and commitment to best practice in Occupational Health and Safety.

The Case Manager Support Worker will review referrals and support with intake arrangements for foster care placements with the guidance of the Home Based Care Case Managers and Home Based Care Coordinator, complete Looking After Children (LAC) Care Plan for the child/young person with the support of the Home Based Care Case Manager, provide support to carers and families with the support of the case manager. This will include participation in reflective practice and training for staff in areas of trauma and its impact on the young person, collaborate with the Case Managers in the Home Based Care Team, and Principal Practitioner in the Therapeutic Team, in developing strategies to support foster carers and ascertain needs of protective placements, maintain an up to date knowledge of the foster care system and departmental requirements of the out‐of‐home care service system, develop and maintain skills key specialist areas such as attachment disorders, parenting styles, childhood trauma, participate in a team approach to service delivery and actively support the Sanctuary model in all aspects of the work in line with the seven Sanctuary commitments; Open Communication, Nonviolence, Emotional Intelligence, Social learning, Democracy, Social Responsibility, and Growth and Change, constructively participate in supervision with the Home Based Care Coordinator. Participate in appropriate professional development opportunities and performance appraisal, foster a spirit of cooperation with open communication, deep listening, and conflict resolution skills, and willingness to learn, maintain confidentiality and privacy of foster carers, children and families, undertake appropriate case recording and caregiver assessment reports with the support of the Home Based Care Case Managers and Home Based Care Coordinator, apply the principles of OHS as prescribed in MacKillop Family Services policy and procedure documentation, report all incidents or Quality of Care events to the Program Coordinator and Case Manager for the child/young person.

This is a Part‐time (22.8hours per week) opportunity based in Forest Hill, VIC within our Home‐Based Care Program. The position reports to the Home‐Based Care Coordinator.

Key Selection Criteria

To be successful in this role, the Case Manager Support Worker will:

* An appropriate tertiary qualification in Psychology, Social Work or a related discipline is essential. Post Graduate training in relevant child, adolescent and family clinical practice, family work and systemic thinking is strongly preferred.
* Evidence of a well‐developed understanding of Attachment and Trauma theories, the relationship to child abuse and neglect, together with an understanding of intellectual disability and an ability to assist others to understand the implications.
* Excellent communication skills, both verbal and written.
* Excellent interpersonal skills, with a demonstrated capacity to work collaboratively with others and exercise influence in a diversity of contexts.
* Ability to participate collaboratively and constructively within teams of peers, staff and other stakeholders, as well as initiative and effective personal judgement when working alone.
