Our team is seeking a dedicated and compassionate Social Worker to join our multidisciplinary team. As a key member of our healthcare team, you will be pivotal in the coordination of patient care to ensure optimal health outcomes for elderly clients in the community.
Key Responsibilities:
• Provide high quality social work service to patients, clients, consumers and their families/carers to ensure the provision of optimal health outcomes consistent with relevant policies, procedures, standards, legislation, and code of conduct.
• Maintain social work and case management responsibility for a number of cases until discharge.
• Early psycho-social assessments, care planning, education and advocacy for the needs of clients, families and/or carers.
• Liaise with community agencies, the client's General Practitioner, counselling and carer support.
• Work as part of an inter-disciplinary team providing low intensity therapeutic home-based programs to frail elderly clients ready for discharge from sub-acute hospital care.
Requirements
• Applicable vaccination requirements for a Category A role.
• Social Work degree and eligibility for the Australian Association of Social Workers (AASW).
• Valid NSW Driver's Licence.
