Job responsibilities
* Provide patient and family education related to the plan of care and discharge needs.
* Perform focused reassessments and escalate concerns through appropriate clinical channels.
* Work 48 hours per week with 12‐hour shifts.
* Administer medications and treatments as ordered and monitor patient response.
* Support safe transitions of care, including handoffs and care plan updates.
