Job Description
We are seeking a highly skilled and motivated professional to join our Adult Case Management team. As a Case Manager, you will be responsible for providing support to adults with intellectual disabilities/autism and their families. Your role will involve serving as an advocate, ensuring choice and satisfaction with services, and monitoring the individual's health and safety.
Our ideal candidate will have a strong understanding of the system of care in Maine and possess excellent communication and interpersonal skills. You will work closely with a multi-disciplinary team to identify needs and develop a plan of care, initiate referrals and monitor services, while coordinating and advocating for services identified in the plan.

Required Skills and Qualifications
Our ideal candidate will possess the following qualifications:
* BA/BS in human services or a closely related field, or BA/BS in an unrelated field + minimum 1 year experience in adult IDD services
* Knowledge of the system of care in Maine preferred
* Reliable vehicle with proper insurance
* Valid driver's license issued by state of residence with clean driving record
