Based Health Group was founded by two Occupational Therapists who believed allied health could be done a little differently - with high clinical standards, genuine team culture, and real care for the people we serve. We have grown carefully across NDIS, aged care, and community home care services throughout Perth, choosing the right people and building strong foundations.
Our Dianella clinic hub is now open and reflects exactly who we are: a purpose‐built space with clinic rooms, a fully equipped rehabilitation gym for participants, clients, and staff, a proper team office with an outdoor area, easy on‐site parking, and a coffee machine you will actually use. Staff have access to the gym during set hours. We are proud of the tight‐knit team we have built - experienced, kind, and genuinely excellent at what they do. We share, we work together, and no one is left to figure things out on their own.
We are building a team defined by kindness, professionalism, and a high level of empathy - people who hold themselves to a high standard and treat everyone, participants and colleagues alike, with genuine care.
About the role
This is a primarily clinical role for a Senior OT who leads from the front through the quality of their work. You will carry a strong, complex caseload across NDIS, aged care, community home care, and Support at Home (SAH), including higher‐risk participants and complex physical presentations. It is a hybrid role with genuine autonomy - we are looking for someone driven to grow, who manages their own caseload confidently and sets the standard for those around them.
Independent, high‐quality report writing is essential. You will bring a strong understanding of the NDIS system, functional capacity, and the home care and Support at Home frameworks, and apply it with minimal oversight. This role has real growth potential and is genuinely incentivised - as the organisation expands, opportunities in clinical leadership, service development, and specialty areas will continue to open up for the right person, with reward structured to match what you achieve.
Full time and part time arrangements are both available.
What you will do
* Maintain a strong, complex clinical caseload across NDIS, aged care, community home care, and Support at Home (SAH)
* Manage higher‐risk participants and complex physical presentations with confidence and sound clinical reasoning
* Deliver Functional Capacity Assessments, Home and Living assessments, Assistive Technology applications, and equipment prescription to a high standard
* Write independently - producing well‐reasoned, planner‐ready clinical reports without requiring significant oversight
* Complete home modification assessments and support participants to access appropriate AT and environmental supports
* Engage confidently with complex psychosocial presentations including mental health conditions, ABI, trauma, and dual diagnoses
* Apply a strong understanding of the NDIS system, functional capacity, and home care/SAH frameworks across your caseload
* Support and guide junior therapists through informal supervision and day‐to‐day clinical mentoring
* Contribute to clinical systems, documentation standards, and service quality as we grow
Who you are
* Someone whose values come first - kind, empathic, and genuinely caring in how you work with people
* Experienced in complex NDIS work, including higher‐risk participants, complex physical presentations, and psychosocial, mental health, ABI, and behavioural needs

* Strong across the NDIS system, functional capacity, and home care/SAH frameworks, or genuinely motivated to deepen this expertise
* Competent in Assistive Technology and home modification assessments
* Driven to grow, motivated by incentive, and energised by raising the standard of what you do
* Professionally mature, reliable, and consistent in the quality of your work
* Supportive and approachable with junior therapists - you mentor naturally, not because you have to
* Someone who brings positive energy, takes pride in high standards, and wants to grow within a team that does the same
