THE ROLE
You'll be the primary point of contact for a caseload of Home Care Package customers across the Bendigo region assessing their needs, coordinating their services, managing their budgets, and visiting them regularly to make sure their care is genuinely working for them.
* Conduct intake, initial, and ongoing assessments of aged care-related needs
* Develop and maintain goal-directed care plans within each customer's home care budget
* Coordinate and facilitate supports and services across the Home Care Packages program
* Manage package spend and advocate for customers' needs and preferences
* Conduct regular home visits and ongoing monitoring across the Bendigo region
* Build strong relationships with customers, families, carers, and community stakeholders
ABOUT YOU
You bring clinical or case management foundations, genuine empathy, and the ability to work independently across a regional caseload.
* Qualification in Case Management, or an undergraduate qualification in Nursing, Allied Health, or Social Work (essential)
* Prior case management experience, particularly in home care or community services (highly regarded)
* Experience completing initial and annual customer assessments and care planning
* Knowledge of the Home Care Packages program and the broader home care sector
* Division 1 Nursing registration (desirable but not essential)
* Based in the Bendigo region with a current driver's licence and willingness to travel across the area
* National Police Check (or willingness to obtain)
* Proof of working rights in Australia
