As part of a multidisciplinary team of Child and Family Health Nurses and Social Workers, you will contribute to the Out of Home Care (OOHC) Health Pathway Program—an important NSW Government initiative aimed at improving health outcomes for children and young people in care. This long‐term contract role allows you to collaborate with DCJ, NGOs, Youth Health PARVAN, Child and Family Health, Paediatric, Mental Health, and Allied Health teams, while benefiting from travel across various sites in the District.

Are You the Right Fit?

We are looking for someone who thrives in individual and autonomous practice while enjoying the safety and support of a team environment. In this role, you will facilitate and implement health assessments and Health Management Plans for children and young people entering Out of Home Care. You will work confidently within a multidisciplinary and multi‐agency approach, collaborating with key stakeholders both within SWSLHD and externally, including DCJ, Department of Education & Communities, GPs, private health providers, NGOs, and carers. Strong clinical, liaison, implementation, and monitoring skills are essential, along with a passion for improving health outcomes for vulnerable children and young people.

Selection Criteria
* Current Authority to Practice as a Registered Nurse with Australian Health Practitioners Regulation Agency (AHPRA). At least 3 years relevant full‐time equivalent post‐registration experience and approved post‐graduate nursing/midwifery qualifications in a field relevant to the specialty field, or such other qualification, or experience deemed appropriate by the organisation.
* Demonstrated currency of practice and competence in assessing children and their families in relation to their health and social needs including an understanding of Out of Home Care and the Children and Young Person's Act.
* Demonstrated experience and knowledge working collaboratively as a member of a multidisciplinary team and with other service providers including Child and Family nurses, Allied health staff, General Practitioners, Child and Family Health Tertiary referral services and negotiation skills necessary to build and maintain collaborative relationships with a diverse range of stakeholders and to work effectively within a multidisciplinary team environment.
* Applicants must possess an Australian C‐Class driver's licence (including P1 or P2). Those who hold a C class licence (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) licence within three months of appointment. Learner's licences are not permitted.
