Conscious Care Group Pty Ltd is a boutique NDIS provider specialising in Supported Independent Living (SIL) across the South-East Queensland region. Following a period of strong growth, we are excited to invite applications for a Service Delivery Manager (SDM) to join our Leadership Team.
This is a key leadership role reporting directly to the Operations Manager, with responsibility for leading Mental Health Support Workers and SIL House Leaders, ensuring high-quality, compliant, and person-centred service delivery.

About the Role
The Service Delivery Manager plays a critical role in operational leadership and NDIS compliance across SIL services, including SIL, STA/MTA and SDA environments.
You will be responsible for:
* Leading and supporting a team of 10+ Support Workers and House Leaders
* Rostering and workforce coordination
* Conducting and attending stakeholder and participant meetings
* Completing risk assessments and providing weekly/fortnightly stakeholder updates
* Organising and coordinating staff training (PBSP, Restrictive Practices, etc.)
* Reporting Restrictive Practice actions to the NDIA
* Maintaining strong NDIS compliance across services
* Supporting positive culture, accountability, and quality outcomes
What We're Looking For
To be successful in this role, you will bring:
* Demonstrated experience in the NDIS sector, particularly SIL
* Strong leadership and people-management skills
* Sound understanding of NDIS Practice Standards and compliance requirements
* Excellent communication and stakeholder engagement skills
Mandatory Requirements:
* NDIS industry experience
* Current QLD Blue Card
* Yellow Card or NDIS Worker Screening Check (NDISWC)
