What you'll do
* Provide personalised support to older individuals in accordance with their unique needs and preferences.
* Assist with activities of daily living, including personal care, domestic tasks and activity support.
* Foster a positive and inclusive setting that promotes independence and community engagement.
* Follow and contribute to the customer's care plan.
* Maintain and ensure completion of documentation, while maintaining confidentiality.
About you
* You are a talented and passionate Support Worker.
* You want to make the most of your expertise, do what you do best and thrive on the challenge of providing our customers with quality care.
* Excellent written & verbal communication skills.
* A passion for supporting choice and independence.
* High level customer service experience.
* Ability to work independently and as a part of a team to support the well-being of customers.
* Willingness to travel between customers' homes and attend site base training.
* Administrative and computer skills.
* Ability to operate a smart phone.
Required qualifications and checks
* Certificate III in Individual Support, Aged Care or equivalent.
* Current First Aid Certificate including CPR.
* Current Police Check suitable for working as a Support Worker in Aged Care (less than 12 months old).
* Current Driver's Licence and access to own vehicle.
What to expect
* You will be reimbursed for kilometres travelled at the SCHCADS Award rate, and you will be paid for the time you travel between customers after your first visit until your last.
* Your first day will be practical training with colleagues.
* Your Team Leader will support you with tools, such as the mobile phone you will be issued.
* You will have buddy shifts with an experienced Support Worker.
* You will have access to Team Leader and Scheduling and Customer Service teams for support.
