We are currently recruiting on behalf of a highly regarded not-for-profit organisation committed to providing responsive, compassionate and person-centred care to older Australians.
We are seeking an experienced Registered Nurse with a strong Care Advisory background within Home Care Packages (HCP) and experience supporting the transition into Support at Home (SAH) to join a passionate and supportive team in a full-time permanent opportunity based in Melbourne's north‐east.

This role is ideal for a clinically experienced and compassionate professional who enjoys building relationships, coordinating care and empowering older people to remain independent and achieve their goals.
Key Responsibilities
* Conduct comprehensive clinical assessments and develop tailored care plans
* Provide clinical oversight and coordinate safe, high‐quality care delivery
* Monitor and review care plans in line with changing client needs
* Collaborate with GPs, allied health and multidisciplinary teams
* Ensure accurate documentation and compliance with HCP and SAH requirements
* Provide education, guidance and support to staff, clients and families
* Promote client independence, wellbeing and person‐centred outcomes
Key Requirements
* Bachelor of Nursing and current AHPRA registration
* Experience in a Care Advisory or community aged care role
* Strong understanding of HCP and transition to SAH frameworks
* Knowledge of aged care systems and funding models
* Understanding of challenges experienced by older people, families and carers
* Demonstrated commitment to person‐centred care practices
* Excellent communication and stakeholder engagement skills
* Ability to work collaboratively within multidisciplinary teams
