About the role
The Clinical Care Coordinator is responsible for leading and coordinating high-quality, person-centred care in line with best practice and organisational standards. Working closely with the Care Manager, this role ensures residents have up-to-date Care Plans tailored to their individual needs and oversees their implementation with the support of a multidisciplinary team. This position reports to the Care Manager or General Manager to ensure effective coordination of resident care, including
* Assist with the admission of new residents, ensuring comprehensive assessments are completed.
* Assess individual resident needs and develop person-centred care plans in collaboration with healthcare professionals.
* Lead and support a dedicated care team to deliver safe, high-quality clinical care.
* Maintain accurate and up-to-date clinical documentation and ensure compliance with regulatory standards.
* Provide hands-on nursing care, including wound management, medication administration, and monitoring.
* Communicate effectively with residents, families, and healthcare providers to support positive outcomes.
* Oversee infection control measures, clinical audits, and quality improvement initiatives.
* Act as a role model and mentor, supporting and developing nursing and care staff.
Essential Requirements
We are looking for someone who brings:
* Bachelor of Nursing
* Current Australian Health Practitioner Regulation Agency (AHPRA) registration
* Nursing experience within a residential aged care facility,preferably at least 2 years of aged care experience.
* Experience of working cooperatively with multi-disciplinary care teams
* Strong clinical assessment, documentation and practice skills
* Effective communication and interpersonal skills
* Proficient computer and information management skills, including ability to use Microsoft and clinical software systems.
* Experience in a similar clinical care role
* An understanding of the aged care accreditation process
