**Position**: Care Coordinator/Nurse
**Location**: Melbourne, VIC (Hybrid - Work on the Road and Work from Home)
**Employment Type**: Permanent Casual (15-20 hours per week)
**Salary**: Competitive, based on experience
**Reports To**: Health Care Services Manager and Directors

**The Role**:
As a Care Coordinator/Nurse, you will play a pivotal role in ensuring our clients receive the highest standard of care. You will be responsible for developing and implementing personalised care plans, providing direct nursing care, and maintaining strong relationships with clients and their families. This role also involves compliance management and interdisciplinary collaboration to ensure a holistic approach to care.
Permanent role.
**Key Responsibilities**:
- **Onboarding New Clients**: Conduct comprehensive initial assessments to understand the unique needs and preferences of new clients. Develop tailored care plans and guide clients through the onboarding process, ensuring they feel supported and informed from the start.
- **Care Coordination**: Continuously implement, and regularly review personalised care plans to meet the unique needs of each client. Adjust care plans and needs of care recipients as per incident reports and allied health reports.
- **Nursing Duties**: Provide direct nursing care such as medication management, wound management and more; conduct health assessments, and educate clients and their families on health management.
- **Client Management**: Build strong relationships with clients, perform initial assessments, and address any concerns promptly as their first point of contact.
- **Compliance and Documentation**: Ensure all care activities comply with NDIS and Aged Care standards. Maintain accurate and up-to-date client records.
- **Team Collaboration**: Work collaboratively with other nurses and team members as well as external stakeholders to provide integrated care.
- **Business Development**: Promote Miracle Health Services and build relationships with potential clients and referral sources.
**Qualifications**:
- At least Certificate IV in Aged or Disability Care with assessment modules, preferably EN or RN with current AHPRA Registration.
- Detailed understanding of different funding programs (NDIS, HCP, DVA, CHSP etc.), would be advantageous.
- Current First Aid and CPR certificates.
- Covid 19 Infection Control certificate.
- Working with Children Check.
- Current National Police Check.
- Experience in healthcare, NDIS, Aged Care, in-home aged care, clinic or similar environment.
- Valid driver's license and willingness to travel.
