About the Role
This is a community-based position, allowing you to work collaboratively with clients both in their homes and in Residential Aged Care Facilities. You will use your clinical skills and knowledge to conduct holistic assessments with residents and clients, focusing on wellness and reablement.
* Capturing individual client information and working collaboratively to develop support plans tailored to their unique goals
* Supporting older Australians to continue living independently by considering supports such as assistive technology, social support services, and community care
