We are seeking an experienced and passionate Palliative Care Nurse Practitioner to join the East Hume Regional Palliative Care Consultancy Team. This senior clinical role provides advanced assessment, diagnosis and management of patients with life limiting conditions across different local government areas including Wodonga, Alpine region, Benalla, Indigo, Mansfield and Towong both in the inpatient and community spaces. The role aims to assist with clinical advice and support for hospital staff and community teams, that are located within these regions, with patient assessments, clinical care and advice around management of complex symptoms through collaboration, supervision, advice and support.

You will deliver expert, person‑centred care, manage complex health needs, prescribe and implement therapeutic interventions, and contribute to improved health outcomes and reduced hospital‑associated risks for older people.
