More than 1000 people choose to work at Fronditha Care, caring for elders across our 5 residential facilities in Melbourne and Newcastle, in their homes and in the community, across metropolitan Melbourne.

The Community Services team coordinates care and services for clients who are living in their homes, supporting them to live as independently as possible and connected to their community. The team oversees and supports an extensive portfolio of clients, through a diverse range of programs. Care and services are underpinned by a holistic assessment of clients' needs and the identification of their goals.

The Care Partner Support Officer is the primary contact for a Support at Home (SAH) clients or their representatives, responsible for the assessment of care needs, identification of care goals in collaboration with clients, development of care plans and co-ordination of service delivery to ensure expenditure is within budget. The Care Partner ensures program activities are in accordance with the Aged Care Act, SAH program guidelines and the aged care quality and strengthened standards.

Requirements


* Establish rapport with clients and their representatives.
* Manage a dedicated case load of clients, meeting all monthly, quarterly, and annual review requirements.
* Review and recommend person-centered care plans, human rights based, trauma aware, healing informed and reflecting the reablement approach
* Identify eligibility for funding:
* Palliative care
* Assistive technology- home modifications
* Restorative Care
* Seek clinical advisory support to ensure the clinical needs of the client are met and regularly reviewed.
* Identify changes in health and wellbeing by reviewing care needs and services.
* Home Visits- Meet and Greet, six monthly monitoring, annual reassessment and other visits as required.
* Complete assessments of client home environment and responds to any identified hazards.
* Maintain current and accurate client records and case notes.
* Identify vulnerable clients and ensure their care plan reflects needs associated with vulnerability.
* Maintain professional boundaries with the clients and their representatives.

About You
* Bachelor or equivalent qualification in health-related field or allied health is preferred
* Qualifications in Community Services or equivalent qualification
* Business Accumen, budget handling and spending, excel and judgmental skills
* Community aged care experience – including knowledge of the issues facing older people or people with disabilities and their carers
* Demonstrated understanding of Strengthened Aged Care Standards and best practice in community aged care
* Excellent communication, problem solving, influencing & negotiation skills, including a capacity to facilitate complaint resolutions
* Thorough understanding and ability to support compliance with WHS, Equal Opportunity and other professional & legal requirements which affect day-to-day care and other services for clients
* Ability to work independently and as a team.
* Ability to prioritize and be flexible to attend to changing workplace demands
* Strong interpersonal skills
* Ability to work within the scope of your role while ensuring all KPI requirements are met.
* Well-developed PC skills
* Experience working with a culturally diverse workforce.
* Experience providing care to clients and residents with culturally diverse needs.

Desirable

Greek language skills an advantage.
