We are seeking a compassionate and reliable Community Support Worker to join our Home and Community Care team in the North Metropolitan area of Perth.

As a Community Support Worker, you will play a vital role in supporting older Western Australians to live safely and independently in their own homes.
">
You will provide personal care and everyday support that empowers clients to maintain their dignity and comfort.
">
This includes assisting with showering, dressing, grooming, and continence support, as well as supporting clients with daily living activities such as meal preparation, light cleaning, and laundry.
">
You will also be responsible for providing social support and companionship to clients, including driving them to shops, appointments, or community activities.
">
Key Responsibilities:
">
* Provide personal care such as showering, dressing, grooming, and continence support in a way that maintains dignity and comfort
* Assist with medication, ensuring clients receive the correct medication (full training provided)
* Support with daily living by offering a helping hand to high-care clients with activities that make everyday life easier
* Lend a hand at home with domestic duties, light cleaning, laundry, meal preparation, and keeping living spaces safe and welcoming
* Be a friendly face by providing social support and companionship, including driving clients to shops, appointments, or community activities
* Follow personalized care plans to make sure each client's needs and preferences are met with consistency and care
* Encourage independence by supporting clients to do as much as they can for themselves, using our wellness approach
* Be part of their day by sharing a chat, a smile, or a listening ear that makes all the difference

Requirements:

To be successful in this role, you will need:
">
- Previous aged care or community care experience, including personal care (showering, dressing, continence support)
">
- Certificate III in Aged Care (or equivalent) – desirable
">
- A current Australian driver's license and your own reliable car
">
- A genuine passion for supporting older people to live independently in their own homes
">
- The ability to build positive, respectful relationships with clients and their families
">
- Good communication skills and a caring, patient nature
">
- The ability to work independently while being part of a supportive team
