Our Clinical Care Partner role ensures clients receive person-centred, high-quality support by overseeing and coordinating complex care needs through assessment, care planning and coordination of services.
* Collaborate with clients and multidisciplinary teams to develop and review comprehensive care plans
* Facilitate seamless communication among healthcare professionals to monitor and respond to changing client needs
* Continuously evaluate care plans, services and outcomes to ensure optimal results
* Assist clients in navigating the care system and connecting them to necessary resources
This position involves regular community engagement and travel.
The ideal candidate will possess:
* Registered Nurse or Allied Health professional holding current AHPRA registration and unlimited work entitlements
* Proven understanding and expertise in client-centred, community-based care services delivery
* Experience and knowledge in Community Funding streams and programs
* Exceptional communication and clinical problem-solving skills
* Current Australian Driver's Licence
* Roadworthy & insured vehicle for work use
* Ability to manage time effectively, including autonomous work arrangements
