About you
* Tertiary qualifications in Nursing or Allied Health (with relevant registration). Desirable.
* Previous Case Management Experience (Community and Aged Care experience highly desired).
* Experience working with clients conducting HPC / STRC assessments and applying goal‑oriented, outcome‑focused and holistic approaches.
* Demonstrated ability to develop and maintain strong networks with internal and external stakeholders.
* Current driver's licence and be comfortable traveling throughout the region each day.
* COVID‑19 Vaccination.
