Transitional Care Support Role
This role focuses on assisting individuals in their recovery following medical treatment or rehabilitation.
* Personal care assistance, including showering, grooming, toileting, and mobility support
* Providing medication prompts and wellness checks to ensure the client's health and safety
* Support with light household tasks such as laundry, meal preparation, and cleaning to maintain a comfortable living environment
* Transportation services for appointments and community activities, ensuring safe and reliable travel
* Communication with care coordinators and family members to provide updates on the client's progress
* Monitoring of the client's wellbeing and identifying potential risks during recovery
The successful candidate will have evidence of current vaccinations against Vaccine Preventable Diseases (VPD) and adhere to health and safety protocols.
