Disability Pathway Navigator
This role plays a pivotal part in enhancing consumer satisfaction and fostering trust-based relationships within the community.
* Coordinate pathways for individuals with disabilities
* Provide expert guidance on the National Disability Insurance Scheme (NDIS)
* Evaluate and ensure culturally appropriate support is accessible to all
The successful candidate will have a deep understanding of First Nations communities, cultural nuances, and the importance of person-centered care. They will be able to effectively navigate complex systems, build strong relationships with clients, and deliver high-quality support that meets individual needs.
Key Responsibilities:
* Collaborate with stakeholders to design and implement culturally responsive pathways
* Develop and maintain relationships with local service providers and organizations
* Conduct thorough assessments to identify client needs and develop personalized plans
Requirements:
* Relevant tertiary qualification in a field such as social work, health promotion, or Indigenous studies
* Proven experience working with First Nations communities and individuals with disabilities
* Excellent communication and interpersonal skills
* Ability to work independently and as part of a team
