Job Title:
Case managers are responsible for applying evidence-based case management practices to individuals at medium or high risk of reoffending.
* Utilize case management practices to actively engage individuals and ensure responsive, targeted interventions to assist in reducing their risk of reoffending.
This requires effective communication, interpersonal, and problem-solving skills to motivate and influence individuals in the case management process.
The ideal candidate will be able to build positive working relationships and work effectively as part of a small, close-knit team.
