About the Role

We are seeking a permanent, 0.3 EFT (22.8 hours per fortnight) Grade 2 Occupational Therapist to join our team within Monash Health Residential Care and our Transition Care Program (TCP). We are happy to negotiate hours and days of work to fit in to your schedule. This role will be based predominately within our Residential Care Homes, located at the brand new Boollam Boollam Aged Care Centre, located in Heatherton and Chestnut Garden, located in Doveton.

This is a unique opportunity to join our large, friendly community-based team. This role will suit an enthusiastic, dynamic and skilled clinician, keen to take responsibility for ensuring the delivery of safe and high-quality client care, driving service development, and championing impactful continuous improvement activities and research.

About You

To ensure your success in this role you will bring the following demonstrated skills, experience and knowledge:

* Demonstrates clinical skills and expertise in the area of community care and gerontology
* Ability to work autonomously and collaboratively with the multidisciplinary team to deliver safe consumer- focused care, providing specialist knowledge or depth of experience

* Provides holistic and comprehensive assessments and interventions

* A well-developed understanding of and ability to navigate complex service systems including (but not limited to) My Aged Care, State-wide Equipment Program and GEAT2GO

Qualification & licence requirements include:

* Current Victorian drivers licence

* Current and Satisfactory Aged Care Clearance (Police Check/Aged Care Worker Screen)

* NDIS workers check
* No current Aged Care Banning Orders in place (as per Register of Banning Orders)
* Registered with State-Wide Equipment Program
* Registration with GEAT2GO
