As a Clinical Nurse Specialist 2 Child and Family Health (CNS2CFH), you will act as an entry point to the service, supporting an integrated multidisciplinary service response based on individual client and family needs.
Your responsibilities will include:
- Acting as a resource person and leader for child and family health staff, with a focus on vulnerable families and their children.
- Attending professional development opportunities and further developing skills and knowledge in child and family health.
- Disseminating information and current best practice relevant to child and family health staff and managers.
- Participating in the provision of education relevant to child and family health.
- Participating in health promotion and promotion of child and family health nursing services.
- Advising and participating in appropriate early intervention, assessments, and referral to services and programs.
- Participating in the development and review of policies and clinical practice.
- Contributing to care coordination and case management of vulnerable families and their children.
- Providing support to child and family health managers and CNC.
- Building and maintaining relationships with service providers and establishing effective referral pathways.
- Collecting and reporting on service events as required, maintaining confidentiality and accurate health care documentation.
To be successful in this role, you will demonstrate:
- Current authority to practice as a Registered Nurse/Midwife with the Australian Health Practitioner Regulation Agency (AHPRA) and successful completion of post-registration qualifications in Child and Family Health Nursing.
- A minimum of 3 years full-time equivalent recent experience working in Child and Family Health Nursing and current accreditation as a Nurse Immuniser.
- Demonstrated commitment to actively contributing to own and the Child and Family Health Nursing streams career development.
- Outstanding verbal and written communication skills, organisational skills, interpersonal, consultation and negotiation skills, and proficiency in use and understanding of computer software and activity and program reporting required for Child and Family Health Nursing.
- Demonstrated sound knowledge and extensive experience of ‘Working in Partnership with Parents’ and the ability to engage and work autonomously with vulnerable families.
- Demonstrated ability and willingness to lead clinical nursing practice development and service delivery.
- Demonstrated ability to work effectively and collaboratively both independently and as part of the multidisciplinary health care team.
- Evidence of a current Class C driver's license and ability and willingness to travel for work purposes.
