We are seeking a dedicated and compassionate individual to join our team as a Support Worker. The successful candidate will be responsible for providing high-quality support services to individuals with disabilities, promoting their independence and well-being.
Key Responsibilities
* Maintain accurate and up-to-date client records, ensuring that all relevant information is documented and communicated effectively to the team.
* Develop and implement personalized support plans that cater to the unique needs and goals of each client, fostering a person-centered approach in all interactions.
* Build and maintain strong relationships with clients, families, and other stakeholders, demonstrating exceptional communication and interpersonal skills.
* Collaborate with colleagues to ensure seamless service delivery, adhering to organizational policies and procedures.
* Possess the flexibility to work varied shifts, including mornings, evenings, weekends, and sleepovers, to meet the needs of clients.
Requirements and Qualifications
* Hold a minimum Certificate III in Disability or Individual Support, or equivalent qualification.
* Obtain and maintain a current Disability Clearance/NDIS Worker Screening Check.
* Possess a valid First Aid Certificate (HLTAID003 & HLTAID001).
* Hold a current Manual Handling Certificate.
* Have a valid Assist with Medication Certificate.
* Possess a current Driver's License.
* Be eligible to work in Australia.
