Salary: RN Level 2: $94,336 - $105,961AHP Level 2: $98,566 - $109,808(Pro-Rata + Superannuation + Leave Loading + Salary Packaging)
We are seeking a Clinical Care & Coordination (CCC) Clinician with demonstrated experience working with clients with severe and persistent mental illness. The successful candidate will have experience with mental health assessments and therapeutic services, including psycho-education and focused psychological interventions / strategies.
Responsibilities
As a Clinical Care & Coordination Clinician you will play a key role in the care of people with severe mental illness, by delivering therapeutic mental health services and coordinating integrated care. The CCC Clinician will collaborate, consult and liaise with primary healthcare providers, acute health, emergency services, rehabilitation and support services, family, friends, and carers and/or other agencies to support optimal client outcomes.
Qualifications
Candidates with relevant qualifications in Social Work, Occupational Therapy, Mental Health Nursing and Registered Psychologists are encouraged to apply.
Relevant tertiary qualifications in a health-related discipline, such as Social Work, Occupational Therapy, Mental Health Nursing and Registered Psychologists.

The successful applicant will be remunerated in accordance with FocusOne Health's Enterprise Bargaining Agreement (EBA) dependent on skills, experience, and qualifications.
