About the Role
As a Lifestyle Facilitator (Support Worker), you will provide tailored supports that promote independence, inclusion, and meaningful community participation for people living with disability.
Your role will focus on:
* Building life skills across social, recreational, employment, and daily living activities
* Encouraging personal choice, autonomy, and meaningful engagement
* Delivering supports using a person-centred and strengths-based approach

Skills & Experience
* Experience supporting individuals with disabilities and/or mental health conditions
* Confidence supporting Participants with complex needs and behaviours
* Strong communication and interpersonal skills
* Experience working active overnight shifts
* Competency in manual handling tasks
* Ability to mentor, guide, and support growth in others
Required Certifications
* QLD Green P-Plate or Open Driver’s Licence
* Fully comprehensively insured vehicle
* Current Blue Card (Working with Children Check)
* NDIS Worker Screening Check (Yellow Card)
* Completion of the NDIS Worker Orientation Module
* Current First Aid and CPR certification
* Proof of two doses of COVID-19 vaccination
Desirable (Not Essential)
* Qualification or experience in Disability or Community Services
* Experience in Supported Independent Living (SIL)
* Familiarity with Positive Behaviour Support Plans
* Understanding of privacy and confidentiality principles
* Ability to build and maintain strong relationships with external service providers
Key Responsibilities
* Support Participants to achieve goals outlined in their NDIS Plans
* Implement Positive Behaviour Support strategies
* Assist with personal care (showering, toileting, dressing)
* Perform manual handling tasks (e.g., hoist transfers, wheelchair support)
* Facilitate community and social participation
* Assist with household tasks including meal preparation and cleaning
* Collaborate with families, allied health professionals, and support networks
* Adhere to all abbaCare policies, procedures, and industry standards
