You will establish, monitor and coordinate patients' treatment plans, utilising virtual technology, in partnership with our home visiting provider team.Use your clinical, people and communication skills to manage exceptional patient care, enhancing and promoting quality of life.QualificationsAbout youYou can work across a 7-day rotating roster between the hours of 10pm and 7am, on a part time (minimum 0.6 FTE).
You will be excited to apply your clinical skills in a energetic work environment, in addition to:Bringing 2-3 years acute hospital experience as a Registered NurseCurrent unrestricted AHPRA registrationExperience in case management and care coordinatorProficient computer skills to enable fast and accurate data entry into patient electronic medical records.Experience working amongst a multidisciplinary teamStrong clinical reasoning and assessment skills.Hold current First Aid and CPR certificatesHold a South Australian Working with Children Check and National Police Clearance (or are willing and eligible to obtain).
