The Details:

Your Role:
* Case Management: Conduct assessments, develop care plans, and support clients on their recovery journey.
* Community-Based Care: Provide mental health services in home, clinical, and community settings.
* Crisis Intervention: Deliver immediate support to individuals in distress, ensuring safety and stability.
* Collaborative Approach: Work alongside a multidisciplinary team, including psychiatrists, social workers, and primary care providers.
* Advocacy & Support: Educate clients and their families, connecting them with resources for long-term care.
What We're Looking For:
* 5+ years of experience in mental health case management
* Current AHPRA registration as a Registered Nurse (Clinical Nurse / Clinical Nurse Consultant)
* Ability to work autonomously in regional & remote settings
* Passion for delivering high-quality mental health care to diverse communities
