Young People Transition Support Specialist
This role focuses on equipping young people (aged 15+9 months to 21) with essential skills and networks to successfully transition into adulthood from out-of-home care.
* Case manage a number of young individuals who are transitioning from care, providing individualized support and guidance.
* Engage in assertive outreach and support strategies that cater to varying levels of intensity, addressing the unique needs of each client.
* Motivate, encourage, and empower clients to address areas of personal difficulty, promoting resilience and self-confidence.
* Collaborate across programs to provide comprehensive support to young people throughout their journey, from intake to longer-term housing options.
* Advocate for the establishment and maintenance of positive family relationships, recognizing their vital role in supporting young people's well-being.
Required Skills and Qualifications
* Working knowledge of the child protection system and the complexities surrounding young people transitioning from care.
* A demonstrated understanding of social and political issues impacting young people, enabling informed and effective support strategies.
* Proven experience and knowledge of case management frameworks and practices, ensuring high-quality service delivery.
* Valid Victorian Driver's Licence, Working with Children Check, Police Check.
* Minimum requirement: a Diploma of Community Services or other relevant tertiary qualifications and/or experience suitable for the role.
