About the Role
As a Social Worker, you will be instrumental in empowering individuals living with psychosocial disabilities to achieve their goals and improve their quality of life. Working within a multidisciplinary allied health team, you will provide both coordination and direct support to clients and their families.
Your responsibilities will include:
* Delivering psychosocial (mental health) support and assessments
* Preparing plan review reports and biopsychosocial/family assessments
* Developing and implementing individual and budget plans
* Collaborating with allied health professionals to coordinate support services
* Supporting individuals and families experiencing social disadvantage
* Providing coordination of supports or specialised support coordination

Key Requirements:
* Bachelor’s Degree in Social Work
* AASW Registration (or eligibility for registration)
* Strong verbal, written, and interpersonal communication skills
* Excellent time management and organisational skills
* Active listening skills and empathy
* Valid Working with Children Check, NDIS Worker Screening, Police Check (within 6 months), Driver’s Licence, car insurance, and a reliable vehicle
* Proficiency in Microsoft Office (Word, Excel, Outlook)
Desirable:
* Experience with clients with dual diagnoses, forensic disability, or forensic mental health
* Knowledge and experience working within the NDIS framework
* Experience working with culturally and linguistically diverse (CALD) communities
* Ability to speak Filipino languages (e.g., Tagalog) is desirable but not essential
* Willingness to travel locally as required
