Transition Care Program – Commencing 2026 (Casual Roles)
We are seeking qualified Enrolled Nurses (ENs) and Personal Care Workers (PCWs) to join our new Transition Care Program, commencing 2026, on a casual employment basis.
About the Transition Care Program
The Transitional Care Program provides goal-oriented, time-limited, and focused care to support older people following a hospital stay. The program aims to help clients regain independence and return home, rather than prematurely entering residential aged care.
Services include:
* Nursing support
* Personal care assistance
* All services are designed to maintain and improve physical and cognitive functioning.
The program supports:
* Adults aged 65 years and over
* Aboriginal and Torres Strait Islander clients aged 50 years and over
* Clients with an age-related illness
Essential Requirements
For Personal Care Workers:
* Certificate III in Aged Care, Disability, or Individual Support
For Enrolled Nurses:
* Current AHPRA registration as an Enrolled Nurse
For All Applicants:
* NDIS Worker Screening Check (NDIS Workers Screen Card)
* NDIS Orientation Module Certificate
* Current First Aid & CPR
* National Criminal Record Check (Aged Care)
* Current mandatory vaccinations apply (NSW Health -Covid -19, Hepatitis B, Influenza)
* Current driver's licence with comprehensive insurance
* Reliable vehicle
* Ability to work within a multidisciplinary, client-centred care model
