About the Role
As the Carer Support Planner – Triage, you'll be the first point of contact for unpaid Carers, often in distress, needing immediate advice, connection, or support. You'll use your calm, professional, and person-centred approach to assess needs, resolve queries at first contact where possible, and connect Carers to services across Carers SA, our partners, and the wider community.
You'll work across multiple communication channels (phone, email, web chat, provider referrals) to triage referrals, prioritise urgent needs, and ensure a timely and effective response. Your strong knowledge of service systems and emotional intelligence will help vulnerable individuals feel heard, supported, and empowered.
Key responsibilities (not limited to)
* Deliver compassionate first-contact support, including triage, referral, and service navigation
* Respond promptly to multi-channel enquiries and after-hours logs
* Complete accurate Carer intake, registration, and strength-based assessments using Carer Star
* Prioritise urgent requests (e.g., emergency respite)
* Support the Carer Support Planning team during periods of high demand
* Maintain accurate records and adhere to confidentiality protocols
* Collaborate with internal teams and external partners to support Carers
* Participate in extended hours roster (between 8am–8pm) as required
About You
You're calm under pressure and thrive in dynamic environments. Your strong communication skills, empathy, and problem-solving mindset allow you to support Carers with compassion and confidence.
You will bring
* Experience in triage, assessment, or telephone-based support services
* Strong knowledge of community service systems and referral pathways
* A person-centred, trauma-informed, and culturally responsive approach
* Excellent time management and CRM proficiency
* The ability to handle complex or urgent situations with professionalism
* A tertiary qualification in Social or Community Services (Cert IV or higher) or equivalent experience
* Current Working with Children Check and National Police Clearance
